HealthTeq Platform
The Connected Care Coordination solution drives effective care coordination, caregiver workflow, and self-management. Our HIPAA-compliant platform automates remote monitoring data collection and integrates seamlessly with EHR systems via HL7/FHIR.
Proven Outcomes in Diabetes & Hypertension
Mean A1c reduction at 6 months (diabetes)
Systolic BP reduction at 6 months (hypertension)
Compliant platform with real-time data transmission
The HealthTeq HIPAA-compliant platform automates collection of remote monitoring data from commercial devices used or worn by patients. We have incorporated wearables and remote monitoring devices from a number of commercial vendors including FDA 510(k)-authorized devices and wellness/fitness devices. Data are transmitted in real-time and managed via the HealthTeq Portal and integrated with EHR via HL7/FHIR interfaces. Patients view their own data with summary graphs and charts of progress over time in myHealthTeq.
Initial published research showed exceptional outcomes. Participants with diabetes achieved a mean A1c reduction of 4.19 (SD 2.69) at six months and reductions in systolic blood pressure of 13.55 mmHg (SD 23.31) at six months.1
In another study with 221 participants, those with hypertension achieved a mean systolic BP reduction of 20.24 mmHg (95% CI: 13.61, 26.87) at six months, while those with diabetes achieved a mean A1c reduction of 3.85 points (95% CI: 3.73, 4.88).2

Five integrated components that assure usability, easy implementation, and seamless interoperability — built to fit your setting and workflows.
Primary clinician & program interface
Portal is the primary interface for clinicians and programs, enabling automated and customizable processes. It provides no-code, configurable dashboards and forms as well as a standards-based interface to exchange data with electronic health records systems or repositories — making program startup and maintenance easy and cost-effective.
Maia is a human-centered, AI-based tool that enables collaborative medication reconciliation and adherence. It optimizes daily self-management, reduces caregiving burden, and prevents adverse events by combining artificial intelligence with a compassionate multi-lingual voice interface.
We leverage AI and machine learning to automate the creation of a comprehensive medication inventory, create an optimized medication taking schedule, and present the schedule as a virtual pillbox — making the process easy for the user.
AI and machine learning automate the creation of a comprehensive medication inventory, removing manual burden from patients and caregivers.
Creates an optimized medication-taking schedule presented as a virtual pillbox — making the process intuitive and easy for users.
Maia, our multi-lingual voice assistant, provides in-app and voice reminders at the designated time and confirms doses taken to fill in the medication record.
Maia delivers the medication inventory and medication record to a healthcare provider or pharmacist designated by the user or organization.
Health Tequity distributes 4G cellular-connected devices — no broadband or WiFi required at participant homes. Devices are pre-configured and connect to the platform out of the box.
4G cellular-connected for real-time hypertension monitoring without WiFi
Cellular glucometer for seamless diabetes monitoring and A1c management
Cellular-connected scale for tracking weight changes in heart failure and high-risk patients
Continuous oxygen saturation monitoring for COPD and respiratory care
We use BLE-Connect to manage the integration of any Bluetooth remote patient monitoring device — pairing, capture, encryption, and transmission. Connect your preferred devices into the platform, whether in myHealthTeq or integrated into third-party apps.
Devices can be shipped to your organization or direct-shipped to a participant's home.
The myHealthTeq app has integrated functionality to deliver surveys and forms directly to participants for symptom tracking, health assessment, or patient-reported outcomes (PROs).
With this capability, an organization can deliver validated health assessment instruments or patient-reported outcomes such as those from national sources like PROMIS — or locally-tailored surveys. Participants can complete symptom reports on a daily or weekly basis, tailored to the health condition being monitored.
Validated Health Assessment Instruments
Deploy nationally-recognized instruments like PROMIS directly to participants
Locally-Tailored Surveys
Create and deliver custom surveys specific to your program's needs
Symptom Tracking
Daily or weekly symptom reports tailored to the health condition being monitored
Integrated Clinician Dashboard
Data summarized and surfaced in the Portal for immediate clinical action
Analytical Database Delivery
PRO data delivered to an analytical database for program evaluation
Condition-Specific Monitoring
Frequency and content tailored to each participant's clinical profile
Quickly implement a chronic illness remote patient monitoring solution designed with rural community health centers — their providers, staff, and patients.
Focus on care, not technology.
We fully support the technology so you can focus on the activities that drive effective chronic illness care coordination and patient self-management.
